1- The presence of the concerned individual.

Every Lebanese citizen requesting a passport should be personally present at the regional office to which his register or residence belong to, after filling the application form of the passport and certifying it  by the Quarter Mayor of registry or residence, except for the minors who are below seven years old and whose presence is obligatory before the Mayor.

In case the travel permit for minors below seven years is signed by both parents before the Mayor, then one of them can present  the application request to the General Security center. But if the travel permit is signed by one of the parents, then the presence of both of them is obligatory at the General Security center in order to present the application request.

2- The exemption from attendance.

Ø  Disabled and permanently ill persons at home or at the hospital are exempted from being present personally according to a written request presented by the concerned person, one of his relatives or the administration of the institution which takes care of him to the regional office or department with a medical report enclosed under the liability of the physician of the hospital who organize the report and then the request is submitted to decide upon.

Ø  In case of approval, the fingerprint of the concerned person is taken in order to complete the application request and to arrange the passport. 

Ø  The regional office will hand over the passport to the concerned individual upon its completion.

3- Filling the application request of the passport

The application is to be filled by the concerned individual according to the remarks written on the application request specimen. A functionary at the office or department might be asked for clarifications when necessary.

3.1-The part concerning the person requesting the passport. 

Ø The person requesting  the passport has to fill the fields specified or assigned for him with a blue ball-point pen or by a typewriter in a clear and legible writing without any diminution, in both Arabic and foreign languages. The documents are enclosed by a paper clip ATTACHE without bending them to the application request and without folding or curling them at the risk of returning the application request.

Ø No application request presented is accepted after a month of its registration at the Quarter Mayor of registry or residence.

Ø As for mentioning the familial status concerning the females: / single, married, divorced or widowed/ it is done in accordance to the wish of the concerned female.

Ø The specified fields for the minors are to be filled by one of their parents, the legitimate trustee or the legal attorney who hold an explicit proxy or power of attorney for such purposes and he has to sign in the specified field before the Mayor.

3.2- The part concerning the Mayor’s job.

Ø The mayor has to verify the authenticity of the documents, as well as matching its data with the data provided by the applicant of the passport.

Ø The mayor has to certify, sign and seal the photograph of the applicant of the passport after mentioning its owner's name, his register number and address, without sticking it to the application request.

Ø He has to fill the specified fields reserved for him, to write down personal information of the two witnesses without the need to enclose copies of their personal documents, then to take their signatures or the fingerprints of their right thumbs  in case they are not capable of writing.

Ø He certifies the authenticity of the information stated in the application, after mentioning his name, his signature and stamp it with his seal.
