1- The individual presence of the citizen
· Every Lebanese citizen requesting a passport should be personally present at the regional office of his residence or civil register address, his passport application form should be filled and certified by the JP (*) of registry or residence, except for minor applicants who are below seven years and whose presence is only obligatory before the JP (*).
· In case the travel permit for minors below seven years is signed by both parents before the JP (*), then one of them is entitled to present the application to the General Security center. But if the travel permit is signed by one of the parents, then the presence of both of them is obligatory at the General Security center in order to present the application.
2- Exemption from attendance
· Disabled and permanently ill people staying at home or at hospital are exempted from being present personally according to a written request presented by the concerned person, one of his relatives or the administration of the institution which takes care of him to the regional office or department with a medical report enclosed under the liability of the physician of the hospital, the request is then submitted for decision.
· In case of approval, the fingerprint of the concerned person is taken in order to complete the application request and to proceed with the passport.
· The regional office will hand over the passport to applicant once ready.
3- Filling the passport application form
The application is to be personally filled by the applicant according to the directions mentioned on the application form specimen. A functionary at the office or department might be asked for clarifications when necessary. For further information the applicant can seek assistance from General Security personnel in the regional office.
3.1- The part concerning the person requesting the passport
· The person requesting the passport has to fill the fields specified or assigned for him with a blue ball-point pen or by a typewriter in a clear and legible writing without any diminution, in both Arabic and foreign languages. The documents are enclosed by a paper clip ATTACHE without bending them to the application and without folding or curling them at the risk of returning the application.
· An application presented a month after its registration at the JP (*) of registry or residence will not be accepted.
· As for mentioning the social status concerning the females / single, married, divorced or widowed / it is done according to the wish of the concerned female.
· The specified fields for the minors are to be filled by one of their parents, the legal guardian trustee or the legal procurator holding an explicit procuration entitling him to such procedures and he has to sign in the specified field before the JP (*).
3.2- The part concerning the JP's job
· The JP has to verify the authenticity of the documents, as well as matching its data with the data provided by the applicant of the passport.
· The JP has to certify, sign and seal the photograph of the applicant of the passport after mentioning the name of the person, his register number and address, without sticking it to the application.
· He has to fill the specified fields reserved for him, to write down personal information of the two witnesses without the need to enclose copies of their personal documents, then to take their signatures or fingerprints of their right thumbs in case they are not capable of writing.
· He certifies the authenticity of the information stated in the application, after mentioning his name, his signature and stamps it with his seal.
* JP: Justice of the peace or commissioner of declarations , it is the judicial officer elected in each district authorized to witness and sign statutory declarations and provide various application forms for official procedures.
