Individual passport:
· Passport Application Form.
· A copy of the applicant’s passport which is intended to be replaced.
· A copy of the new ID card or a copy of the individual civil registration extract issued less than ten years ago or a copy of the concerned person’s passport issued in Lebanon after 1990.
· Two recent colored photographs, showing the face completely on a white background, size 3.5 cm × 4.3 cm (as shown in the diagram attached)
Family Passport:
In addition to the required documents mentioned above, the applicant must include:
· A family civil registration extract containing the names of accompanying minors to be added to the passport.
· Two recent photographs of the full face of each minor accompanying, measuring 3.2 cm × 4 cm (as shown in the diagram attached) |

· The charge for passport applications in the diplomatic delegations abroad is set according to the current regulations. |
· The application is filled within the delegation in presence of the applicant, who should sign it before the competent person, who should also sign it along with the Head of the Mission. In exceptional cases the application can be filled before a Notary Public or a Honorary Consul (in countries where there is no diplomatic representation or there is a large distance separating the place of residence of the Lebanese person and the Lebanese Mission).
· The applications are sent by diplomatic pouch with the documents required by the Lebanese missions abroad to the Department of the Lebanese passports directly via the Ministry of Foreign Affairs and Emigrants – General Security Section.
· Upon delivery of the accomplished passports to their respective owners the old passports are canceled by the missions.
· The mechanism of treating the passport applications to replace a damaged or lost passport by the missions abroad and the procedures are performed according to the current regulations.

All copies of documents attached to the application must be clear and declared authentic by the Lebanese Mission for the confirmation of compliance with the original document.
