· An Identity Card.
· An extract of Civil Registration.
· A Ration card or a certificate from the UNRWA stating that he does not have a record in its registry.
· An identification and residence certificate issued by the JP (*) along with the testimony of two witnesses including the photocopy of their identity cards.
· Three colored photos stamped by the JP’s seal.
· 60.000 L.L. for the period of one year.
· 180.000 L.L. for the period of three years.
· 300.000 L.L. for the period of five years concerning the travel document only.
· The damaged travel document or the clear parts of it should be presented.
· In case of total damage the application is presented on the basis of a lost travel document.
· The applicant will be subject to an investigation process in the General Security Regional Office where the application is presented in order to clarify the causes of the damage.
· Once the approval to issue a new document replacing the damaged one is granted, the individual attends the regional department along with all the required documents in order to obtain a new travel document.
* JP: Justice of the peace or commissioner of declarations , it is the judicial officer elected in each district authorized to witness and sign statutory declarations and provide various application forms for official procedures.
