Procedure

 

·   Placing an announcement of loss of the travel document or pass in a daily newspaper for three consecutive days.

·   Presenting an application for the replacement of the lost travel document or pass ten days after the last newspaper announcement in the General Security Regional Office along with the following documents:

-  An identity card or individual extract of Civil Registration of the applicant.

-  One copy of each daily announcement in the newspaper.

-  The statement of the applicant in the regional office where the application is presented clarifying the circumstances of the loss.

·   Once the approval is granted, the individual attends the regional office along with the approval in order to obtain a Palestinian travel document, with the following required documents:

-  An Identity Card.

-  An extract of Civil Registration.

-  A Ration card or a certificate from the UNRWA stating that he does not have a record in its registry.

-  An identification and residence certificate issued by the JP (*) along with the testimony of two witnesses including the photocopy of their identity cards.

-  Three colored photos stamped by the JP’s seal.

 

Fees

 

·   60.000 L.L. for the period of one year.

·   300.000 L.L. for the period of five years concerning the travel document only.

 

Notes

 

·   The damaged travel document or the clear parts of it should be presented.

·   In case of total damage the application is presented on the basis of a lost travel document.

·   The applicant will be subject to an investigation process in the General Security Regional Office where the application is presented in order to clarify the causes of the damage.

 

* JP: Justice of the peace or commissioner of declarations , it is the judicial officer elected in each district authorized to witness and sign statutory declarations and provide various application forms for official procedures.
