·   The applicant’s Identity Card.

·   The applicant’s extract of Civil Registration issued by the Palestinian refugees' administration in Lebanon.

·   A certificate from the UNRWA stating that the applicant is registered even if his name is mentioned on his parents' ration card or a certificate that he does not have a record in its registry.

·   A statement of travel permit from by his parents or his legal guardian.

·   Three colored photos stamped by the JP’s seal.

·   An identification and residence certificate issued by the JP ^(*) along with two witnesses and the photocopy of their Identity cards.
