Ø  An Identity Card.

Ø  An extract of the Civil Registration issued by the Palestinian refugees' administration in Lebanon.

Ø  A certificate from the UNRWA stating that he is registered even if his name is mentioned on his parents' ration card or a certificate that he is not registered.

Ø  A statement of traveling permit from by his parents or his custodian trustee.

Ø  Three colored photos stamped by the Mayor's seal.

Ø  An identification and residence certificate issued by the Quarter Mayor along with two witnesses and the photocopy of the identity card of each witness.
